Protect yourself, your family, and patients from the flu today!

A Joint Labor-Management Partnership agreement and Influenza Vaccination/Masking Policy requires all Kaiser Permanente Northwest employees to get an annual flu shot or, if they decline, wear a mask in patient care areas after the mask-on date is announced. The mask-on date will be announced once the flu season has been declared, which could happen at any time.

To indicate that you have received your flu shot, you’ll need to present a record of your influenza immunization to your manager and receive a green dot.

To produce your record, logon to KP.org, select My Medical Record, click Immunizations, and print a paper copy; or, provide documentation from your primary care physician; or, written verification from Military service, a non-Kaiser organization, or other pharmacy. Redact all Personal Health Information (PHI) except for the current flu vaccine.

Here’s a flyer that you can refer to with more details: Have you gotten your flu shot and green dot?
