Kaiser PPE Concern Escalation Form

If you receive a mask that doesn't fit properly (like size, ear loops, or nasal bridge problems) or if the construction is flimsy and tears, you should request a different mask to protect yourself from droplet exposure.
Steps to take to get a replacement:
  1. Ask your greeter for an alternative mask. (Be kind – your greeter is a fellow union member.) If that doesn't fix the problem...
  2. Ask your manager/HAS/D.A. for a replacement. If your manager does not comply quickly...
  3. Tell your steward and complete this PPE concern escalation form.
  4. If the problem is serious or ongoing, file a complaint with Oregon OSHA or Washington OSHA. You must do this from a computer.
