What is a Grievance?
A written grievance is a formal complaint by a union member who believes the employer has violated our union contract.

How Do I File a Grievance?
If you think you have a grievance, contact your union steward so they can work with you through the process.

Check the contract to determine key information such as which article has been violated and the timelines for filing.

Gather all the information: Who, What, When, Where, and How did the incident occur?

The Workplace Investigation Form may be helpful for making your notes on the incident and being sure you've documented everything necessary. You can use this form to organize your information before you talk to your steward or organizer.

Be sure to contact your union steward and discuss your concerns. Keep in mind that there may be other ways to address your issue without filing a grievance. Your union steward can help you with ideas and advice about whether a grievance is the best option in your situation.

 

How Do I Fill Out the Form?

Click the link (listed below) to download the form and save it to your computer.  The document will open in a new tab. You will not be able to hit the print button or fill in the form in this viewer.

Move your curser to the top and click "open with a different viewer" or click the the arrow pointing down to begin your download. It will look something like this:

The form will begin to download or a dialogue box will open prompting you to "save". Click "OK" to save it. If prompted, choose a location on your computer to save it to. Do not open the file and begin filling out the form without first saving, or you are likely to lose your work.

 

Fill Out by Hand
If you want to hand-write your grievance, open the dowloaded file and click on the gray “Print Form” button at the top right corner. Remember, only a brief description of the issue is needed. Do not include your arguments in the description.

Enter the information about your grievance, and then fax or scan and e-mail the form to the OFNHP office (ContractAdministration@ofnhp.org; 503.657.7456) and to the manager.

 

Fill Out on the Computer
If you want to type your grievance, open the downloaded file. It will open now as a fillable form. Rename the file for your new grievance.  Begin filling the form. Be sure to save as you go.

Remember, only a brief description of the issue is needed. Do not include your arguments in the description.

Save your final version and email the finished form as an attachment to both the manager and to ContractAdministration@ofnhp.org. (You can also print and fax.)

I Filled Out the Form. Now What?
When we receive your form, we will check it for completion and call or email you if we have any questions.

We will then send an email courtesy acknowlegement of the grievance to the manager, copying union staff, steward, and grievant.

If you do not hear from us within 2 days, either with questions for you or with the email to your manager, please call the office at 503.657.9974 to make sure we received your form.
