If you used your Kaiser Vision Benefits at Vision Essentials between February 1 and April 26, 2016, you may have been eligible to receive a larger discount and may now be due a refund of a portion of what you paid.
If you used the percentage discount and received 25% you will be reimbursed the difference between that discount and the 40% discount you should have received. If you used your percentage discount benefit you will automatically receive a reimbursement for the difference and do not need to respond to this letter or take any additional action.
If you applied your $150 benefit to your qualifying purchase you may be due reimbursement for the difference between what you paid and what you would have paid if a 40% discount had been applied to the purchase instead of your $150 benefit. Kaiser Vision Essentials has committed to correcting these errors as well, but you have only 30 days to submit your claim for reimbursement.
What steps do I need to take to claim my reimbursement?
1) If you used your percentage discount benefit you will automatically receive a reimbursement for the difference and do not need to respond to this letter.
2) If you used the dollar benefit and not the percentage discount and believe you might be due some money back, you will need to contact claimmydiscountkpnw@gmail.com.
You will need to provide your name, contact number, employee identification number and current address.
3) There is a limited time in which you can make a claim for reimbursement. Submit your claim NO LATER THAN Friday, March 31, 2017.
When can I expect to see my reimbursement:
Once the period for submitting claims for reimbursement closes at 5pm on Friday March 31st, 2017, Vision Essentials will have 45 days to review claims and verify information.
If you were not eligible for the 25% discount, but received it, you will not be eligible for the 40% discount. This refund program applies to items purchased for the employee only; it excludes purchases made by family members. If you already received a refund relating to purchases made during this time you are not eligible for another discount based on the same purchase.
